The Community Access Service Coordination provides the following: 

• Determination of service eligibility for state and local services including day programs, residential programs, and other community supports  
• Advocacy for all service needs including educational advocacy for children and adolescents receiving special education services  
• Assistance in the development of an individual service plan   
• Monitoring of individual service plans to ensure implementation as well as health and safety of the individual in their home or community setting
