Eligibility for Community Access Service Coordination includes the following:

• Resides or has significant ties to McHenry County
• Sub-average general intellectual functioning (IQ 70 or below)
• Deficits in adaptive behavior
• Deficits manifested before the age of 18 years

• Custodial Paperwork (child)
• Medicaid card
• Birth Certificate
• Social Security Card State ID Card
• Diagnostic information/reports from doctors or other professionals
