Eligibility for Community Access Service Coordination includes the following: - Resides or has significant ties to McHenry County
- Sub-average general intellectual functioning (IQ 70 or below)
- Deficits in adaptive behavior
- Deficits manifested before the age of 18 years

- Medicaid card
- Birth Certificate
- Social Security Card State ID Card
