Community Access Service Coordination…How to Refer 

Eligibility for Community Access Service Coordination includes the following:

Resides or has significant ties to McHenry County

Sub-average general intellectual functioning (IQ 70 or below)

Deficits in adaptive behavior

Deficits manifested before the age of 18 years  

Required Documents : The following is a list of documents we require in order to serve you or your family member: 

1. Guardian Information (if applicable)
2. Custodial Paperwork (child)
3. Medicaid card
4. Birth Certificate
5. Social Security Card
6. State ID Card
7. Diagnostic information/reports from doctors or other professionals
