Under the direct supervision of the Child and Family Connections Program Manager, the Service Coordinator will assist families in navigating the Early Intervention system through intake, Individualized Family Service Plan (IFSP) development, and transition at age 3 years; will link children and families to the necessary services and supports; will serve as a liaison between Early Intervention services for families and community resources; will work cooperatively with CFC staff, EI providers and school district personnel. The Child and Family Connections Service Coordinator will have significant travel throughout McHenry County, with periodic out of county meetings/trainings.
